K-12 Scholarship Instructions

Scholarship funds are available from Swigert Foundation for youth that want to play ice hockey or ice skate, who qualify for assistance, and who are willing to help earn their scholarship by completing reporting requirements.

To be considered, the young person and his/her family must:

Be willing to follow through on the commitment to participate in required practices, games and competitions;
Be willing to abide by the rules and regulations of the game, the facility and requirements of the coaches, and practice good sportsmanship;
Demonstrate the need for financial assistance by sending the front 2 pages of last year’s tax return/1040. You may black out all social security numbers. Please explain any drop in current year income.
During the playing season, you will provide the Foundation with a school or hockey (preferable) picture/snapshot and a copy of school report cards for at least 2 quarters or final school year’s report). At the end of the season, we would like the student to tell us how their season went.

 

New applicants will be considered regardless of past grades. To be considered for continuing assistance the skater must show they are attempting to improve low grades and trying to hold at least a C average.   Grades K-12 applicants are given preference.
 
The attached application must be completed by the student and parent or guardian, signed by both and returned to:

Applicants will be notified of the decision by the Foundation’s review board no later than September 20th. Applicants awarded other scholarships for the season may not qualify.

 

Except in exceptional skill cases, scholarship funds will only cover skater registration fees:

Registration fees will be paid directly to the facility/school/league on behalf of the scholarship recipient.
Equipment or services purchased or rented can either be paid directly to the provider or reimbursed to the recipient’s family upon proof of payment.


The foundation assumes no liability for, but not limited to the following: injuries incurred during participation in or transportation to or from games, practices or competitions; availability or quality of facilities and/or equipment; or for the actions of management, the coaching staff, officials or spectators.

The Foundation reserves the right to choose scholarship recipients based solely on the discretion of the Foundation’s selection panel.
